FORM C, RECIPIENTS OF SERVICE
Student evaluation forms are available from the personnel officer responsible for the maintenance of the Central File in the Personnel Office.
COUNSELORS AND LIBRARIANS
The appropriate vice president, dean, and/or ILS is responsible for developing recipients of service forms with their faculty. Recipients of service forms must specify the same evaluation criteria and procedures for all faculty serving in similar functions. All recipients of service forms must be submitted to the Plan coordinator for approval, who will place master copies in the Central File.
CHAIRPERSONS AND COORDINATORS
The appropriate vice presidents, deans, or ILSs are responsible for developing recipients of service forms that ask respondents to identify a faculty member's strengths and weaknesses in relationship to his or her approved major functional responsibilities (MFRs). All recipients of service should be instructed to complete and send the confidential form to the appropriate ILS by a specified deadline. Signatures are optional. The ILS should compile the responses into a document for distribution to the evaluatee and other evaluators.
OTHER RECIPIENTS OF SERVICE
If a faculty member does not fit into one of the above categories or if the recipients of service form is inappropriate for a particular job function (e.g., instructor of a telecredit course), the dean and ILS should develop an appropriate recipients of service form. All recipients of service forms developed for such functions must be submitted to the Plan coordinator for approval who will place a master copy of the form in the Central File.
