Before actually withdrawing from a course, you should discuss this option with your instructor(s).  There may be other options available.

To withdraw from any or all classes for any cause during the semester, you must do the following:  (1) get a withdrawal or drop form from the Admissions and Records Office; (2) obtain the proper signatures; and (3) return the completed form to the Admissions and Records Office.  Be sure to retain one copy of the form until you have received your end-of-semester grade report.
The last day to withdraw from a full-semester class is shown in the Schedule of Classes each semester and is normally at the end of the twelfth week of the semester.  If you are enrolled in a class that ends earlier than the traditional length courses, you should ask the instructor for the withdrawal deadline.  You may not withdraw after a course has ended or after a Q(uit) grade has been submitted by the instructor.  The official withdrawal date is the date the Admissions and Records Office receives the proper, signed withdrawal form.
If you follow this withdrawal procedure, you will receive a grade of W.  Otherwise, a grade of F will be given.  Failure to attend classes or stopping attendance does not constitute an official drop.

Only when the following two criteria are fully met:  an actual emergency situation arises   and 70% of the course has been completed can you request an Incomplete from your instructor.  It is at the option of the instructor who will, if granted, explain the procedure and the implications of this choice.
